my mhealth app - fully funded access for GP practices in Kent and Medway

The ICB has extended funding for access to the my mhealth digital self‑management platform for a further three years. The platform is available at no cost to GP practices for eligible patients across Kent and Medway.

The service supports patients to self‑manage long‑term conditions through clinically validated digital tools and education resources.

Conditions covered

GP practices can offer access to the following my mhealth programmes:

  • myCOPD - patients aged 18+ with COPD
  • myAsthma - patients aged 13+ with asthma
  • myDiabetes - patients aged 18+ with type 1 or type 2 diabetes
  • myHeart - patients aged 18+ with heart conditions.

Clinical assurance

The my mhealth platform aligns with national guidance and assurance frameworks, including:

  • NICE Early Value Assessment recommendations for cardiac rehabilitation, pulmonary rehabilitation, COPD and asthma self‑management
  • QISMET approval, supporting increased access to diabetes structured education.

Patient access

Patients can self‑register via a text message link or QR code. There is no waiting list, and access is immediate once invited.

Support for practices

Practices can opt into a fully managed digital engagement campaign, delivered by the my mhealth Digital Health Adviser (DHA) team. This includes:

  • SMS/email invitations to eligible patients
  • Secure data sharing via a single DHA Agreement
  • Set‑up and onboarding support

How to participate

Practices wishing to participate should email dha.support@mymhealth.com to request the DHA Agreement and set‑up support. A short set‑up call will then be arranged.
