CTR oversight panel referral form

About the panel

  • The panel includes professionals in health and social care and people with lived experience.
  • The panel meets once every 12 weeks.
  • The panel discusses what is being learned overall from Care and Treatment Reviews regarding care, safety and peoples experiences of hospital and support services.
  • The panel is able to escalate issues to let services in health and social care know about concerns in health, safety and support.

Types of issues the panel will look at

  • Concerns that have been raised about safety, care, support and treatment.
  • If people may have been left alone for long periods of time (seclusion) Why? How often?
  • When there are long delays in getting the right support or in discharge planning.
  • When complaints have not been resolved or things have not improved.
  • How safeguarding referrals are being managed.
  • When people in hospital are declining offerings of a CTR.
  • The quality of care and treatment reviews and how well these are happening.

The panel will not look at

  • Urgent or emergency issues that have only just happened.
  • Issues that are already being looked at - like a complaint or safeguarding alert.

The panel on rare occasions may review individual cases.

This is for autistic people and people with a learning disability who:

  • have had a recent CTR (for example, within the last year)
  • despite having support from professionals and agencies, things are not getting better or care and treatment plans are not working
  • are having difficulties because of problems across different professional agencies and help outside of those agencies is needed to help resolve those problems.

When we receive a referral, a member of the Care and Treatment Review (CTR Team) will make contact to see if the oversight panel is the right the right place for this concern or if it will be addressed better by another service or team.

The panel will not look at issues that are already in process, like a complaint or safeguarding alert that is being investigated.

The panel will need your consent to be involved.
