The A&G feature now allows a multi-way conversation to take place between GPs and consultants that can help strengthen professional relationships, share knowledge and promote important clinician-to-clinician dialogue around patient care. Transforming access to specialist care across Kent and Medway
The Kent and Medway Referral Management Programme (RMP) is a system-wide collaboration bringing together primary care, community services, acute providers and commissioners to create a more efficient, consistent and patient-centred approach to accessing specialist care.
By 2027, the programme will establish a single, digitally enabled Single Point of Access (SPoA) for specialist advice across priority specialties, helping patients receive the right care, in the right place, at the right time.
The programme responds to increasing demand for outpatient services, long waiting times, variation in referral pathways and growing pressures on both primary and secondary care. Through greater use of Advice & Guidance, standardised clinical pathways, community-based options and digital innovation, we aim to improve patient experience while making best use of resources across the health and care system.
Our vision is to create a shared care model where primary, community and secondary care teams work together to support patients without unnecessary delays, duplication or barriers to accessing services.
Requests for Specialists Input will be supported by agreed clinical pathways, rapid specialist input and a multidisciplinary approach that ensures patients are directed to the most appropriate form of care, whether that is self-management support, community services or specialist treatment.
