To make a referral

Self-referral or referrals from a family member are now available for people aged 10-25 in West Kent.

 It will be available for Medway and the rest of Kent later in the year.

  • If you live in West Kent but are over the age of 25
  • Or if you are living in East Kent or Medway

Please contact your social worker or the professional in CAMHS or Mental Health Together to make the referral for you.

  • is 26 and over
  • or if you live in Medway or East Kent

please contact your social worker or the professional in CAMHS or Mental Health Together to make the referral.

Consultation

After we receive your form, one of our professionals will contact you.

They will talk with you to learn more about you and to make sure the services in the Dynamic Support Approach are right for you.

The professional will be a point of contact to help you avoid admission to hospital or in settling back in the community after being in hospital.
