Consent

We need your consent for these services to work with you.

For people under the age of 16; a parent or legal guardian will need to give this consent. They should involve the person in making that decision.

Information about these services should be provided in advance to help you or your family make a choice about these services.

There are some people who may not be able to make this decision. When this happens, a Best Interest Meeting involving those responsible for the person’s care and support will be needed to decide if these services would be helpful.
